Integrated Health and Wellbeing Service

We are one of the few UK organisations offering a fully integrated health and wellbeing service that can be tailored to suit the needs of local communities. Completely flexible in our approach, we have the capability to create a truly bespoke product for each of our clients based on their requirements.

Our integrated health and wellbeing service comprises six core elements:

1. Health and wellbeing hub

Our health and wellbeing hubs are resourced by our health coaches who are trained in engaging, assessing and motivating people to change their health-related behaviours using a non-judgemental and compassionate approach.

The wellbeing hubs, provide a range of behaviour-change interventions and can be accessed via a freephone number and a specially designed website.

After assessing a client’s eligibility and requirements, either online or over the phone, our health coaches will guide them towards relevant support channels addressing health behaviours such as weight management, physical activity, smoking, mental wellbeing and alcohol and substance misuse. Importantly, they’ll consider each client’s holistic needs, including the wider determinants of health such as employment, debt and housing support.

We pride ourselves on our industry-leading, bespoke Customer Relationship Management (CRM) technology, which captures and stores all communication and information relating to each client throughout their health and wellbeing journey and allows dynamic analysis and reporting.

It includes functionality to filter assessments and programmes based on the needs of the individual.

Stand-alone interventions

To complement our health and wellbeing hubs we offer stand-alone support for a number of interventions such as:

  • weight management
  • physical activity
  • alcohol misuse
  • substance misuse
  • smoking cessation
  • mental wellbeing
Programmes are tailored to the requirements of the commissioner and are delivered by specialist practitioners.
2. Community outreach and engagement

Establishing strong links with local communities, with a particular focus on those with the greatest health needs, is vital.

Our outreach teams embed themselves within their community and, with a targeted presence in housing estates, faith settings and community venues, promote self-care behaviours and building resilience. Offering accessible, flexible and person-centred community development, the teams encourage individuals to take responsibility for their own health and wellbeing and support their peers to do the same.

The teams provide high quality, motivational and brief interventions that support health improvements as well as referring individuals onto specific support services such as physical activity and smoking cessation.

3. Specialist training

We provide a range of health and wellbeing courses specifically designed around public health and health promotion. Courses are wide ranging and include subjects such as Health Awareness, Understanding Behavioural Change, Encouraging Physical Activity, Understanding Sexual Health and Understanding the Delivery of an NHS Health Check.

4. Social marketing campaigns

Our high profile, engaging ‘on the street’ health improvement campaigns are designed to achieve maximum reach and impact.

We deliver social marketing campaigns that have the ability to change or maintain people’s health behaviours for the benefit of individuals and society as a whole. Throughout the process, we keep client insight at the heart of our campaign development. Importantly, we’re open minded when it comes to the audience we’re attempting to influence and the behaviour we’re hoping to change or sustain.

All new campaigns include a ‘launch’, to engage key community groups and explain the campaign approach and objectives, ensuring strong buy-in and engagement.

5. Awareness raising and prevention programmes

Our team can devise and implement a wide range of community and workplace-based awareness and prevention programmes, from running or attending health fairs and events to administering health MOTs.

Our programmes are designed to inspire learning and engage their target audience rather than adopt a passive ‘look-and-see’ approach. Topic choices are limitless, ranging from older adult health, women’s health and back health to stress management, nutrition and mental health.

We are equally experienced in delivering suitable programmes for young people in schools, and other educational and youth settings covering topics such as tobacco, alcohol and substance abuse. Knowing that traditional awareness campaigns can sometimes fall short for young people, our ideas focus on programmes that are fun, interactive, challenging and, most importantly, designed to have a more sustainable impact.

Our health check service is custom built to the requirements of the commissioner and can include any of the following measures:
