Becoming more active
We understand that life can be a balancing act and trying to fit everything in can be a challenge, which is why our programme can be taken remotely, supporting a flexible approach to suit your lifestyle. The programmes involve a variety of one-to-one sessions, group workshops and online learning so you’re fully supported for the whole journey.
How does it work?
- You will have 12 weeks of one-to-one sessions with your Health and Wellbeing Coach
- You can choose between having your sessions face-to-face or remotely
- The programme includes online learning so you’re fully supported for your whole journey
How can I sign up?
If you feel that you would benefit from the service, are over 18 years and are registered with a Mid Sussex GP Practice then click the ‘Refer myself’ button below, or alternatively, you can ask your GP, nurse or NHS Health Check professional to refer you.
