A new, innovative and free service for people with Type 2 diabetes: Type 2 Diabetes Path to Remission Programme. A one-year programme to support healthier lifestyle, weight loss, and remission of Type 2 diabetes.

What is it?

  • A unique one-year programme which combines specialist nutrition and physical activity alongside personalised strategies that support the development and maintenance of a healthy lifestyle. 
  • The programme helps participants to achieve rapid and sustainable weight loss. This lowers blood glucose levels which can reduce, and in many cases remove, the need for Type 2 Diabetes medication.
  • Supported by GPs, practice nurses, diabetes practitioners, dietitians, psychologists, and exercise specialists
  • Coaching sessions delivered by a diabetes practitioner, online learning and supporting resources including meal plans and recipes

How does it work?

12 weeks of low-calorie diet (typically soups, shakes and porridges) followed by 6 weeks of gradual food reintroduction and 8 months of weight maintenance, achieved through nutritious meals, movement, and healthy lifestyle behaviours.
