You have been selected for the the NHS Low Calorie Diet Programme

Following a recent referral from your GP, you have been provisionally selected to take part in the new NHS Low Calorie Diet Programme, which is an innovative and free service for people with Type 2 diabetes.

About the programme

The NHS Low Calorie Diet Programme is a unique one-year programme combining specialist nutrition, psychology and physical activity, with the aim of supporting rapid weight loss, long-term behaviour change and remission of Type 2 diabetes. We are proud to share that those who have already joined the programme have achieved some incredible results; with average weight loss in the first 12 weeks exceeding 12kg, and many of our completers achieving the ultimate goal of diabetes remission.  

If you choose to take up your place on the programme, you will begin by spending 12 weeks in our Total Diet Replacement phase, meaning you will replace your usual food with soups and shakes. As part of our commitment to you, all the Total Diet Replacement products you need will be provided to you free of charge when you start the programme. Following the Total Diet Replacement phase, you will transition to focusing on food reintroduction and then onto maintaining your new healthy eating lifestyle. 

As part of this highly sought-after programme, during the 12-months you will receive the following specialist support: 

  • An initial one-to-one assessment 
  • 20 sessions with your Health & Wellbeing Diabetes Practitioner 
  • 39 online support modules 
  • A final one-to-one assessment

Am I eligible?

To be eligible for the programme, you must: 

  • be aged 18–65 years 
  • have had a diagnosis of Type 2 diabetes within the last 6 years 
  • be willing to commit to the full 12-month programme 
  • have any agreed medication changes in writing from your GP. If your GP has recommended making medication changes at the start of the programme, and you don’t have these in writing, please contact your GP to ask for a copy before you contact us to book your place 

Unfortunately, you are unable to take part in the programme if you:  

  • are a current insulin user 
  • are pregnant or planning a pregnancy in the next 12 months 
  • have had or are awaiting bariatric surgery (unless willing to come off the waiting list) 

What should I do next?

As spaces on the NHS Low Calorie Diet Programme are limited, we ask that you contact our Patient Support Team within 14 days of the date of this letter to either book your place on the programme, or to inform us that you would like us to offer your place to someone else. You can contact our Patient Support Team either by telephone (charged at a local rate): 0333 577 3561 or email: query@xylahealth.com and they are available 9am-5pm Monday to Saturday. 

If you are able to commit to the programme and choose to secure your place, you will firstly speak to a member of our Patient Support Team. This booking call will last around 20 minutes, and our friendly team will be able to tell you about the programme, confirm your eligibility and commitment, as well as book your sessions. During this call you will also be able choose your Total Diet Replacement start date. If you already have commitments which may mean starting the programme in the next 3 months isn’t possible, please talk to our team about this. We also understand that not everyone is able to commit to this programme, so if you decide not to take up your place, please do inform our team so we can discharge you back into the care of GP and offer your place to someone else.
