Supported discharge

Our supported discharge service provides a rapid discharge from hospital for patients to return home (or to their appropriate care setting) following an agreed period of personalised, short-term care.

A patient-centred, end-to-end intermediate care solution, supported discharge enables your own service delivery under our CQC registration. We supply nurses, support workers, health coordinators and managers, enabled with rostering, training and day-to-day management.
